What is a Personal Health Record (PHR)?

A PHR is an electronic record of health-related information on an individual that conforms to nationally recognized interoperability standards and that can be drawn from multiple sources while being managed, shared, and controlled by the individual. The PHR will include information that is entered by the individual themselves, as well as medical data from other sources such as pharmacies, labs, radiology, dental, and health care providers.  A PHR will provide a complete and accurate health and medical information, medical summary and medical history. The information is compiled and the information is hosted into an electronic format to assist in the management of their personal health.

Why is a PHR important to You and Your Doctor?

According to the FDA, medication errors cause 1 fatality a day and injure 1.3 million people annually. There is a consensus among stakeholders that the widespread adoption of health information technology will lead to safer, more effective healthcare for the individual. There is a definite need for a system that reveals peoples’ medical information in a timely and user-friendly manner. Unfortunately, many people die from the simple fact that not enough medical information is known about them during emergency treatment, ie. allergies, current medications, etc. In many cases, the patient is not in a position to deliver the information, and the caregiver has no other means of receiving critical medical information. Ultimately, the PHR will allow physicians and their patients to benefit from improved information that can be shared to make the best decisions concerning their healthcare. Care coordination from a process management perspective is critical to improved clinical outcomes.

According to a 2006 report by the National institute of Medicine, more Americans die from medical errors each year than breast cancer, automobile accidents, and even AIDS.

An article in the “USA Today”, (Aug-2007), a study given in
“The Journal of American Medical Association” indicates the medical error death rate has changed little in the past 7 years.

A PHR provides pertinent information such as medication, dosage, frequency, allergies, discharge summaries, and the clinical outcomes of the last doctor visit. The complete PHR can eliminate additional testing by medical care providers and provide/assist with an accurate medical diagnosis at the time of 1st response and emergency room treatment.

What Information is Stored in the PHR?

Personal Health Records contain the following data content:

-           Name, age, address and date of birth
-           Emergency contact information- family-friends
-           Emergency response-Medical alerts-listed allergies, current medications
-           Name, addresses and phone numbers of their physicians, dentists and specialists
-           Health insurance information
-           Benefit information- Medicare/Medicaid
-           Living Wills, advance directives, (DNR), Video Wills, medical power of attorney
-           Organ donor authorization
-           Third party insurance information, so both the individual and provider know their financial responsibility
-           Complete medical discharge summaries-prior illnesses and surgical procedures
-           Current medications and dosage amounts
-           Immunizations history with accurate dates
-           Allergies or sensitivities to drugs and materials, such as latex
-           Important events, dates, and hereditary conditions in their family history
-           Results from a recent physical examination
-           Opinions of specialists-doctors notes
-           Nursing home and home health functional assessments
-           Home health and nursing home admissions
-           Tests results; eye and dental records
-           MRI’s, Radiology results, Cat-Scans 
-           Correspondence between the patient and their provider(s)
-           Special needs information
